WHERE IS THE ONSITE CARE CLINIC LOCATED?

The Boone clinic is located at 188 Milton Brown Heirs Rd, Boone, NC.


The North Wilkesboro clinic is located at 118 White Pine St., North Wilkesboro, NC.

WHAT ARE THE HOURS OF OPERATION FOR THE ON-SITE CLINIC?

The Boone clinic is open 8:00 am – 5:00 pm - Monday-Friday. The clinic will be closed for lunch from 1:00 to 2:00 pm. The last appointment will be at 4:30 pm.


The North Wilkesboro clinic is open 8:00 am – 4:00 pm - Monday-Friday. The clinic will be closed for lunch from 1:00 to 2:00 pm. The last appointment will be at 3:30 pm.

WHO IS ELIGIBLE TO USE THE ONSITE CLINIC?

The clinic is open to benefits-eligible employees, regular part-time employees, and retirees with 10 years of service and were 65 or older at their time of retirement. Plus benefits-eligible family members* may use the clinic. The clinic serves patients from infancy on up. You must have your insurance card and photo ID to be seen in the clinic. If you are unsure of your eligibility to use the clinic please contact HR.

*Benefits-eligible family member is a dependent that is eligible to participate in the UMR health or dental plan (legal spouse or child under the age of 26). If your benefits-eligible family members are not currently covered by the health plan but would like to utilize the clinic, please make sure their information is listed in Workday.
You will see a “to-do” task in your Workday inbox, as shown in the email, to view/add dependents. You will be able to view all current dependents by clicking the orange Dependent button in the Workday task. If additions are not needed, simply scroll down and select “submit” within the Workday task to close. The dependent information must be completed and submitted before the dependent can utilize the clinic.

DOES THE ONSITE CARE CLINIC TAKE WALK-INS?

While the clinic will have a limited number of same day appointments, we HIGHLY recommend that you make an appointment to avoid waiting and to ensure that you are able to be seen.

WHAT HAPPENS IF I’M LATE FOR MY APPOINTMENT?

Patients arriving later than 15 minutes for their scheduled appointment may be asked to reschedule.

AM I ALLOWED TO USE THE ONSITE CARE CLINIC DURING MY WORK SHIFT?

No. You will need to take sick time to go to the clinic. Seek your supervisor’s permission before leaving work.

DOES THE CLINIC OFFER VIDEO OR TELEPHONE VISITS?

WILL ANNUAL FLU SHOTS BE PROVIDED BY THE ON-SITE CLINIC?

Yes, you can get your flu shot at the clinic. Also, be on the look out for annual Flu Vaccine events.

WHAT SERVICES ARE AVAILABLE AT THE CLINIC?

DOES THE CLINIC PROVIDE PRE-EMPLOYMENT DRUG TESTING?

No. The clinic does not provide pre-employment or random drug testing

ARE THERE ANY HOLIDAYS THE ONSITE CARE CLINIC WILL NOT BE OPEN?

The on-site clinics will follow the standard Samaritan’s Purse holiday schedule.

DO I HAVE TO PAY A CO-PAY OR DEDUCTIBLE TO BE SEEN IN THE ON-SITE CLINIC?

No. The clinic’s services are free to anyone eligible to be seen in the clinic, and no co-pay or deductible will apply. The only exception may be some laboratory services. The medical provider will discuss any charges prior to drawing labs (if applicable). The provider may also order an x-ray for you at an in-network facility if appropriate. You will be responsible for the co-pay or other non-covered fees related to any imaging, just as you would be if an x-ray was ordered from any other medical provider office. Please note we strongly encourage you to keep your benefits plan as not all services are able to be performed at the clinic.

DO I NEED INSURANCE TO BE SEEN AT THE ONSITE CARE CLINIC?

No, the clinic is open to benefits-eligible employees, regular part time employees, and retirees with 10 years of service and were 65 or older at their time of retirement. Plus benefits-eligible family members with or without insurance for the majority of services. Some labs or other offsite services may be billed to your insurance plan. For those without insurance, some services may be limited. Please call before coming if you would like more information.

WHO HAS ACCESS TO MY MEDICAL RECORDS LOCATED AT THE ONSITE CARE CLINIC?

Only Wake Forest Baptist Health (WFBH) clinicians providing your care will have access to your medical records. They utilize an electronic medical records system, so information can also be shared with your primary care physician. Samaritan's Purse or its employees do not receive employee information as WFBH is bound by strict privacy and confidentiality laws governed by the Health Insurance Portability and Accountability Act (HIPAA).

WHO ARE THE PROVIDERS AT THE ONSITE CARE CLINIC?

Our clinic is staffed by licensed providers from Wake Forest Baptist Health.

WHAT ARE THE CREDENTIALS OF THE PROVIDERS?

Both clinics will be staffed with a Physician, Nurse Practitioner or Physician Assistant, Medical Assistant and Registered Nurse. Availability of providers is subject to change, please call the clinic with questions or concerns.

MAY I HAVE MY PRESCRIPTIONS FILLED AT THE ONSITE CARE CLINIC?

A limited number of medications are available to be dispensed for same day care at the Boone location for employees & dependents on the UMR insurance plan.

CAN THE PROVIDER AT THE ONSITE CARE CLINIC WRITE ME OUT OF WORK FOR FMLA?

No. The provider at the clinic can only write employees out of work for three days for illness. More serious concerns that may result in longer time away from work will be referred to a primary care physician of the employee’s choice.

I HAVE A PRIMARY CARE PROVIDER.  CAN I STILL USE THE ONSITE CARE CLINIC?

YES! The clinic is not meant to take the place of your Primary Care Physician. If you need a primary care or specialist referral to Wake Forest Baptist Health or any other local provider, a clinic staff member can assist you.

MY PRIMARY CARE DOCTOR IS NOT WITH WAKE FOREST BAPTIST HEALTH.  WILL HE/SHE BE ABLE TO SEE THE RESULTS OF MY LAB WORK OR ANY OTHER VISIT INFORMATION FROM THE ONSITE CARE CLINIC?

YES. Wake Forest Baptist Health works with other local practices to ensure records are shared. This allows our providers to ensure the best care for patients.

CAN I GET INFORMATION ABOUT MY MINOR CHILD THROUGH THE ONLINE PORTAL?

The parent/legal guardian can only message about a minor child through the online portal if there is a valid Proxy Authorization form already signed and in the system.

To have a Proxy Acess form on file, please complete it and return it to mwhstaff@wakehealth.edu.

CAN I REQUEST INFORMATION ABOUT A SPOUSE OR OTHER FAMILY MEMBER THROUGH THE ONLINE PORTAL?

No, to protect the privacy of your family members, we can not give you health information for anyone other than yourself. 

If you have a minor child and would like to access information about them you must complete the Proxy Authorization form and return it to mwhstaff@wakehealth.edu.

CAN I SHARE MY ONLINE HEALTH PORTAL WITH A SPOUSE OR OTHER FAMILY?

No, each adult patient must have their own portal account and can only message their health care provider about their own health.
